# Delivery Health Assessment

A framework for quickly assessing the health of a programme or project, identifying risks, and creating a recovery plan.

## How to use this assessment

Rate each area on a scale of 1-5 (1 = critical risk, 5 = strong health).

### 1. Governance & Decision-Making
- Are roles and responsibilities clearly defined?
- Is there a working governance forum with the right attendees?
- Are decisions being made and communicated in a timely way?

### 2. Planning & Roadmap
- Is there a realistic, up-to-date plan visible to the team?
- Are dependencies identified and managed?
- Is the roadmap aligned with business priorities?

### 3. Resourcing & Capacity
- Does the team have the right skills and headcount?
- Is there a clear understanding of current vs. required capacity?
- Are key risks around attrition or availability identified?

### 4. Stakeholder Engagement
- Are stakeholders engaged and informed at the right level?
- Is there a communications plan in place?
- Is feedback being actively sought and acted upon?

### 5. Delivery Execution
- Are delivery cadences (stand-ups, reviews, retrospectives) working?
- Is progress tracking transparent and accurate?
- Are blockers being identified and resolved quickly?

### 6. Risk & Issue Management
- Is there a live risk register?
- Are issues escalated promptly to the right level?
- Are mitigation plans in place for top risks?

## Scoring

| Score Range | Status | Action Required |
|-------------|--------|-----------------|
| 25-30 | Healthy | Maintain current practices, share learnings |
| 18-24 | At Risk | Develop improvement plan within 2 weeks |
| 6-17 | Critical | Immediate intervention required |

For any area scoring 3 or below, capture:
